I am an Anthem Blue Cross provider, if you have another PPO or POS insurance plan, and/or have a Flexible Savings Account, I can provide you with a statement that you can submit for reimbursement.
Cash and check are accepted for payment.
You must notify me at least 48 hours in advance of your appointment that you cannot come otherwise you are responsible for paying the full cost of the session.
